In Memory of someone special

An “In Memory” donation is a thoughtful tribute to the memory of a loved family member, friend or co-worker. Your generous contribution to Surrey Hospital & Outpatient Centre Foundation will honour their memory by helping patients and families in need of care at the hospital or outpatient centre.



A list of people who have been honoured by receiving an in memory gift are displayed monthly in the Surrey Now-Leader,  Peace Arch News, Cloverdale Reporter and North Delta Leader. See example on the right.

How our In Memory program works

When you make a donation to Surrey Hospital & Outpatient Centre Foundation, the person you designate (ie. next of kin) will be sent a personalized letter. This letter shows in whose memory the gift has been made and indicates your name as the donor. The amount of your gift will not be disclosed.

To make on online gift In Memory of someone special, please click the button above. Or click on the link to print out a donation form that you can mail or fax to the Foundation office. We can also take your donation over the phone.

Cumulative in memory donations of $1,000 or more are celebrated with a listing on our In Memory donor wall for a period of five years.

How to request memorial gifts

The person arranging the funeral or memorial service can instruct the funeral director to insert the following phrase in the obituary notice:

“The family requests that in lieu of flowers, donations can be sent to Surrey Hospital & Outpatient Centre Foundation for (purpose if desired) 13750 – 96 Avenue, Surrey, BC V3V 1Z2 T: 604-588-3371, or made online at”

Convenient postage-free, self addressed gift envelopes are available by calling the Foundation office at 604-588-3371.
